What is self-reporting of disease?

The self-declaration of illness (ADD) is a document that proves that the user is in a situation of illness, declared by him/herself, under oath.

What is self-declaration under oath?

Means that you become yourself user taking responsibility for justifying that he was absent from work because he was sick.

Who can request self-declaration of illness?

Self-declaration of illness can be requested by any worker aged 16 years or older.

When can I apply for self-declaration of illness?

Self-declaration of illness can be requested at a a maximum period of 5 days from the first day of absence due to illness.

For example: if the user needs to prove a situation of illness from Monday to Wednesday, he must request self-declaration between Monday and Friday.

How long can self-declaration of illness be?

Each self-declaration justifies no maximum of 3 consecutive days of absence due to illness. It is up to the user to assess whether or not to use all the days, depending on their state of health.

How many self-declarations can I request?

user can request two self-declarations of illness per calendar year, for a maximum period of 3 days each. That is, the user will be entitled to a total of 6 days (non-consecutive) justified by self-declaring himself sick.

Where can I request self-declaration?

You can request self-declaration of illness:

  • na SNS 24 portal personal area
    • log in via:
      • Digital Mobile Key or
      • Citizen Card or
      • Health User Number
    • click on the menu "I need... Self-declaration of illness"
    • confirm all the data in the order summary window and click "Confirm Order" OR "Close" to go back
    • After completing these steps, the self-declaration will be active
  • na SNS 24 App
    • log in via:
      • Digital Mobile Key or
      • Health User Number
    • go to the "I need..." and click on the "Self-Declaration" item
    • click the "Order" button
    • enter the disease onset date (day, month, year) and click "Validate"
    • After completing these steps, the self-declaration will be active

If digital issuance is not possible, you can also request self-declaration through Line SNS 24 (808242424).

Where can I find the access code for my self-declaration?

You can check the access code for your self-declaration of illness by:

  • from the personal area of SNS 24 portal
  • da SNS 24 App
  • do SMS and/or confirmation email received
  • da SNS Line 24 (808242424)

Do I need to go to a consultation to ask for self-declaration?

No. You will be able to issue a self-declaration of illness without the need to go to your health centre or hospital, or go to a doctor.

Are the days of absence from work with self-declaration paid?

No. With self-declaration of illness, the absence is justified, but not paid.

Similarly to what happens with the Temporary Incapacity Certificate issued by a doctor, commonly known as sick leave, in the first 3 days of absence due to illness there is no payment of remuneration by the employer.

How do I tell my employer that I am absent because I have a self-declaration?

To notify the employer of sickness absence, you must provide the employer with the access code you received via SMS and/or email after the self-declaration has been issued. Subsequently, through the page validate self-declaration of illness it is possible to verify the authenticity of the declaration. 

If I'm sick for more than three days, can I extend self-declaration?

No. If after the 3 days of self-declaration you are still ill, you should go to a doctor for clinical evaluation. If the health professional considers that, due to illness, he or she is still unable to work, the self-declaration will be replaced by a Temporary Disability Certificate, commonly known as sick leave.

For the purposes of granting sickness benefit and so that Social Security is aware of all the days that the user has been absent from work, the user must inform the doctor of the existence of a self-declaration of illness and request a medical leave with a start date equal to the start date of the self-declaration.

The health professional can confirm the existence of the self-declaration in the professional's area in the context of the user.

I'm sick and I've exhausted my self-declarations. What should I do?

If you are unable to attend the workplace due to illness and have already applied for two self-declarations of illness to which you are entitled per calendar year, then should be evaluated in consultation with your doctor.

Is this measure guaranteed in the job code?

Yes. The amendment to Article 254 of the Labour Code was approved by the Assembly of the Republic.
